Who We Are

The LiveWell Kershaw Coalition is the community convener and coordinator around population health improvement in Kershaw County. Its primary function is to coordinate, manage, and oversee the implementation of the Community Health Plan (CHIP) for Kershaw County. The CHIP is a written plan that outlines key goals and objectives to support the improved health of individual residents in our community, based on the Community Needs Assessment. City and County government leaders, non-profits, businesses and residents review, collaborate, and determine what specific role they play in supporting the improvement of health outcomes.

The CHIP workgroups have identified Evidence Based Initiatives (EBI’s) that are now being implemented throughout Kershaw County. LiveWell Kershaw Coalition continues to work to foster collaboration between Kershaw County citizens of all ages in partnership with public and private sector organizations and schools to increase community awareness of health issues. Our ultimate goal is to move the needle toward improved access to quality healthcare while encouraging the people of Kershaw County to work together to take ownership of the health of the whole county.
